The following forms are available here for use by members. Currently we have our various club registration forms available. Please be careful that you use the correct form (each one is explained below). All forms are in PDF format, when you click on the form name, it will launch a new window and present the form to you using Adobe Reader (most PCs will have the reader already installed - if yours doesn't, see the link to Adobe at the bottom of the page). When you are happy that you have the correct form, print it off on your local printer and complete the content by hand. Please make sure that you complete the form as instructed and when sending the form to DSD, that you include any required items as detailed on the form.
- Young Athlete/Parent/Leader-Coach Code of Conduct. All young athletes, parents, and coaches-leaders involved with the club must sign up to the conditions in the relevant sections of the club's code of conduct. This must be acknowledged upon registration each year.
- Juvenile (New Member) Registration Form. This form is for juvenile athletes (under 19 in the relevant year) who have not registered with DSD AC previously. Photocopies of birth certificates are required to be enclosed with this form (please do not send originals of birth certs, photocopies are preferrable).
- Senior Registration Form. This form is for all senior athletes/committee members/coaches/helpers and anyone who is not under 19 in the relevant year.
- Club Gear Order Form. This form is for for all athletes/parents wishing to order club gear. Please print off and fill in this form and return to a member of the parents comittee.
